How travel assignment extensions work: what to know before you say yes

Most travel contracts run 13 weeks, and many units ask you to stay longer. Here is how extensions work, what can change when you extend, and how to decide whether staying is the right call.

What is a travel assignment extension?

An extension is an agreement to keep working at the same facility after your original contract ends, usually for another 4, 8 or 13 weeks. It is a new contract term, not an automatic rollover, so the pay, schedule and terms you have now only continue if they are written into the extension.

When do facilities offer extensions, and how do I ask for one?

Facilities usually raise extensions four to six weeks before your end date, when they know whether the unit is still short. If you want to stay, tell your recruiter early and ask them to request it. Do not wait for the last week, since budgets and paperwork take time.

Can my pay change when I extend?

Yes. Your rate can stay the same, rise or fall, depending on the facility's current budget and demand. Ask for the new taxable rate, stipends, overtime and guaranteed hours in writing before you agree. Our pay explainer shows what a complete package should list.

What else can change in an extension?

Beyond pay, the schedule, unit, shift or guaranteed hours can change. Read the extension like a new contract and compare it with your original terms.

  • Schedule. Confirm shift length, days and weekend or call expectations.
  • Unit. Confirm you are staying on the same unit and floating rules have not widened.
  • Hours. Confirm guaranteed hours and cancellation terms match or improve on the first contract.
  • Housing and stipend. Ask whether your housing situation and stipend rules still fit if you stay longer.

Should I extend or move on to a new assignment?

Extend if the team is solid, the pay is competitive and you are not worn down by the schedule. Move on if the unit is understaffed in ways that hurt you, the pay is slipping or you want a new city or specialty experience. Staying can also save you the time of a new credentialing cycle.

What should I check before signing an extension?

  • The paper. Ask for the extension in writing and read the whole document, not just the dates.
  • Notice terms. Check what notice you owe and what the facility owes.
  • Reimbursements. Ask whether any reimbursement or bonus applies to the extension period.
  • Your next move. Ask your recruiter to line up a backup so you are not scrambling if you decide not to stay.

Questions

What clinicians ask before taking this kind of assignment

How long is a typical extension?

Commonly 4, 8 or 13 weeks, depending on what the facility and you agree to.

Will my pay stay the same if I extend?

Not necessarily. The rate can stay flat, rise or fall, so ask for the new package in writing before you say yes.

When should I tell my recruiter I want to extend?

As early as you can, ideally four to six weeks before your end date, so the facility can approve it in time.

Can I say no to an extension?

Yes. An extension is optional, and declining does not affect your original contract, which you complete as written.

Does it cost me anything to work with you?

No. Placement is free to you. The facility pays our fee.
